Purchase of this product allows for online payment  for the Affidavit of Completion CEU Certificate.

To receive your certificates:

• Fax (616- 897-9401) or email (to Carol@lifepointemedia.com) your Affidavit(s) of Completion. *
• Purchase one CEU Certificate online for each completed course. **
• If you need your certificates right away, include a return fax number or email address.
• Your certificates will be mailed within 3 days to any USA or Canadian address.  For locations outside these areas, your certificates will be emailed as JPG images.

 

NOTE:

*  Affidavits must be filled out completely, including all lesson titles and required signatures.  Please follow the example shown above.

** A completed course is one volume of any Bible series, or any single-volume course.
